In Samuel Hahnemann’s philosophical framework, life is sustained and governed by a spiritual vital force. In Aphorism 12, he explains that this same force is the sole producer of disease.
Disease as a Dynamic Derangement
It is the morbidly affected vital force alone that creates the state we call “disease”. This represents a fundamental shift from viewing illness as a material object or a localized malfunction of physical parts. Instead, the internal dynamis (vital force) becomes deranged, and this internal, spirit-like change is the primary cause of all illness.
Symptoms: The Language of the Vital Force
While the derangement of the internal vital force is invisible in itself, it expresses its suffering through morbid phenomena and alterations that are perceptible to our senses. These external signs—the symptoms—reveal the whole disease to the observer and the physician. Hahnemann asserts that these symptoms are the only way the “Lord of life” has allowed the internal suffering of the organism to be known.
The Standard of a True Cure
The logic of Aphorism 12 provides a definitive standard for recovery: if a treatment causes the disappearance of the totality of symptoms, it necessarily implies the restoration of the integrity of the vital force. When the external signs of suffering are eradicated, the “whole internal and external morbid state” of the vital force is also removed. This process results in the restored health of the whole organism.
Knowledge for Utility, Not Curiosity
In a significant footnote, Hahnemann notes that it is of “no practical utility” for a physician to attempt to understand exactly how the vital force produces these symptoms. He believes this internal mechanism will forever remain concealed from human perception. A true practitioner of the healing art must focus on what is revealed to the senses, as this information is “fully sufficient” to enable a rational and certain cure.

Aphorism 12 reinforces the homoeopathic focus on the observable whole rather than theoretical, hidden internal mechanisms. By addressing the vital force through the evidence of symptoms, medicine moves away from the “absurdity” of materialistic systems and toward a science that confirms its success through the permanent return of health.
